We’re talking new rules, money scene shuffled around, the works. If you’re on Medicare, you’ll notice changes in how you get your meds, what hoops you have to jump through for treatments, and how much you’ll end up paying out of pocket. Prior authorization rules? Shaken up. Drug prices? New models. Some people are calling it a “Medicare earthquake,” which feels dramatic, but honestly, it’s kinda spot-on.

People are buzzing some are pumped about potential savings, others are sweating bullets about what might get messed up. It’s a mixed bag, and nobody’s totally sure how it’ll all land.

1. Out‑of‑Pocket Cap Rises to $2,100

Medicare Part D enrollees will face a higher annual out‑of‑pocket cap beginning in 2026: rising from $2,000 to $2,100. As drug prices climb with inflation, patients could bear up to $100 more annually before the catastrophic coverage kicks in.

2. Prescription Payment Plan

Patients can now spread their drug costs over the year through the Medicare Prescription Payment Plan. Starting 2026, plans will automatically enroll enrollees unless they opt out making monthly budgeting easier. The structure builds upon 2025’s pilot program and now becomes standard.

3. Expanded Anti-obesity Medications

Biden-era proposals to expand coverage for anti-obesity medications (like Ozempic) under Part D were blocked by Trump’s CMS. As a result, medication access for obesity remains restricted preserving before exclusions.

4. Non‑Opioid Pain Treatments

Under the proposed NOPAIN Act, CMS will reimburse specific non‑opioid pain drugs and devices separately in outpatient settings from 2026 through 2027, raising access to alternative pain care.

5. New AI Rules

Strong support on past approval and AI-based denials under Medicare Advantage are set in the new legislation to lower coverage delays and increase transparency. Plans have to specify why claims are rejected and boost appealing procedures.

6. Tech-Heavy Services Cut

CMS is proposing a 2.5% efficiency adjustment to work RVUs for many procedures tied to technology impacting audiology, radiology, and telehealth services.

7. Health Program Cut

Started in July 2025, H. R. 1 (“Big Beautiful Bill”) starts massive cuts reducing Medicare and Medicaid by over $1 trillion over a ten years. Beginning in 2026, these rollbacks are expected to disenroll close to 12 million Medicaid users, reduce funding for safety-net hospitals, and limit Medicare infrastructure.

8. Medicare Advantage Plans

More seniors may be directed into Medicare Advantage plans, which have historically refused more claims and provide less protection than conventional Medicare.

9. Modest Pay Boost for Physicians

After a five-year decline, Medicare fees for doctors will finally rise in 2026 due to updated MACRA-based payment adjustments. Physicians may see small improvements, though larger systemic inflation pressures remain.
